SAMIKSHA Vaccinations Cards

1.SAMIKSHA cards are available at various hospitals, nursing homes, doctors’ clinic and chemist shops.
2.Parents can purchase the cards to avail the various services offered by Samiksha. It will be valid for a period 5 years from the birth of the child.
3.The card holders will receive reminders for their child’s vaccination from time to time via text messages, calls and e-mails from the respective place of purchase of the card.
4.The alerts will be sent along with details of the due immunization, importance of that particular vaccine and other tips.
5.It will enable the card holder to maintain a record of their child’s vaccination schedule. An immunization scheduler will also be provided along with the card for reference.

How To Use Card
1. Scratch the card .There will be a 16 digit code on the card
2.Register with us: Sms this code along with dob of your child to {code/dob} or go online to www.samiksha.com and fill up a form.
3.If you have messaged: You will receive a Call on your mobile number. You will be asked for the following information.
*Your code
*Dob of your child
*Your name or spouse’s name
*Your address and Your e-mail id
4.If you filled in a form online: You will receive a message of registration

Benifits Of Cards
Atter you get registered, your child's vaccination calendar will get created with us. You will begin to receive the alerts as and when required. This calendar will keep updating your child's vaccination dues and the ones taken.
Only once you inform us on your child's vaccine, can we maintain a record. You will receive a call after a few days post the due date and check whether the vaccine taken or not and the date. You may also call and enquire about the record, if you ever need any information about it. Our customer support staff will be pleased to help you out.


Loss of card:
Your registration with us is valuable and we do not wish to lose it but just in case you lose your card, you can call us and inform us. Your code and details will be sufficient.

Change of mobile number:
In case you happen to change your mobile number, you can call our customer support executive on ...- and mention your new number and the update will automatically be made.

Separate card for each child
If you have 2 or more children, you must buy separate cards for both. One card will be valid only for one child. For each child, a separate record will be maintained; hence you will have to purchase a new card for a new child. Each child has different profile. Even in case of twin babies, it is important to maintain different records for them.
